
Hormonal Skin in Women: From Pregnancy to Menopause

In a woman’s life, major hormonal milestones most visibly influence skin: pregnancy/postpartum and perimenopause/menopause. Though these stages may be decades apart, both feature profound hormonal shifts that drive inflammation, collagen synthesis, hydration, circulation, and barrier function.
Many women perceive sudden or unexplained skin changes that are actually foreseeable physiological effects of these hormonal transitions. Knowing the science behind them enables a strategic, not impulsive, approach to skincare and treatments.
Pregnancy & Postpartum Skin: Hormone Surge followed by Hormone Withdrawal
Pregnancy represents one of the most hormonally dynamic periods of a woman’s life. Estrogen, progesterone, and melanocyte-stimulating hormone increase significantly, leading to increased vascularity, pigment activity, and immune changes in the skin.
Clinically, this often presents as increased pigmentation, such as melasma, increased redness from vascular changes, acne or congestion due to hormonal fluctuations, visible vessels, heightened sensitivity, and more reactive skin.
During pregnancy, the best we can do is focus on supporting and protecting the skin rather than correcting it. Many active ingredients, such as retinol, are avoided, as are injectables and laser treatments. Supportive treatments, such as facials, and the use of SPF are encouraged.
Following delivery, estrogen levels rapidly decline. During this time, many women notice increased dryness, barrier disruption, dullness, persistent pigmentation and increased inflammation. Skin may also feel thinner or more fragile. From a physiological standpoint, this reflects increased transepidermal water loss and reduced collagen stimulation as estrogen levels fall.
Postpartum is where we begin thinking about restoring skin function before moving toward correction. After pregnancy and delivery, the skin is often in a compromised state due to hormonal withdrawal, inflammation, physiologic stress and of course, sleep disruption. During this time, the skin barrier is often weaker, hydration levels are reduced, and inflammatory pathways may remain elevated.
For this reason, my clinical approach is to first focus on rebuilding skin strength. This means prioritizing hydration, restoring barrier integrity, calming inflammation and improving circulation before introducing more corrective treatments.
